STRAY – STRAY WAIT, HOLD FOR LOST&FOUND Reason STRAY
Intake condition EXAM REQ Intake Date 01/22/2017, From NY 11354, DueOut Date 01/25/2017,

Medical Behavior Evaluation GREEN
Medical Summary S/O: Initial exam. BAR on presentation. Patient was tolerable of all handling and social. scan negative intact male Approx. 6 yrs. mm = p/m; CRT< 2 sec. EENT = Thick, cloudy opacification of the lens, OU – no conjunctivitis, discharge, or scleral hyperemia; clean AU, NND, mild dental tartar. H/L = nma, nsr, PSS; clear lung fields bilaterally ABD = snp, nmp GU = MI, 2 testicles descended INTEG = mild matting throughout body, unthrifty/dirty coat. Small (2-3 mm in diameter), round puncture wound over dorsal thoracic area, just caudal to the left scapula, and initially covered with dried blood and fur. Wound oozed mild bloody discharge. Skin surrounding wound appeared mildly inflammed. LN = wnl CNS= A+A – PLR present OU, Absent Menace OU MS = 4-5/9; Ambulatory x 4 A: Middle aged MI Yorkie with Mature cataracts OU – r/o inherited vs. geriatric vs.nutritional vs. 2ary to underlying disease (diabetes) Wound – r/o ruptured pustule vs trauma vs. other Matted Px: Good P: Recommend blood work +/- ophthalmalogical assessment. Continue to monitor while in BACC

PROFILE:

Basic Information:

Gizmo is a Yorkshire terrier who was found as a stray and brought to ACC.

Socialization:

Around strangers Gizmo is friendly, outgoing and plays gently with children 4 months and older. Gizmo did not spend any time with any other cats or dogs while with his caretaker so it is unknown how he will react to being around them.

Behavior:

Gizmo did not attempt to bite/scratch any other animals or humans while with his care taker. Gizmo was not bothered by loud sounds. Gizmo is friendly when someone touches his food or bowl while he is eating, takes a treat or toy away, pushes/pulls him off of the furniture, holds/restrains him or disturbs him while he is resting or sleeping. Gizmo’s caretaker did not attempt to brush him, give him a bath, or trim his nails so it is unknown how he will react to those events. If an unfamiliar person approaches his family or home Gizmo is friendly toward them.

For a New Family top Know:

Gizmo is described as friendly, affectionate, playful and mellow with a medium activity level. When at home Gizmo will follow around his caretaker and does not usually play with toys. Gizmo was kept indoors only and was fed dry food. Gizmo is house trained and rarely has any accidents. Gizmo was not left alone and was not crate trained. Gizmo knows the command sit and “go outside”. For exercise Gizmo likes slow walks on a leash.

Intake Behavior:

Gizmo had a loose body and a wagging tail during intake and would lay down on his back for belly rubs. Gizmo allowed staff to pick him up and hold him with no issues and allowed staff to collar, scan for a microchip and take pictures.
