LVT intake: Microchip Scan:positive  Observed Behavior: tense and allowed handling but quickly escalated and would snap at handler, attempted to flee when removed from transport van  Sex:    Estimated Age:     Subjective: matted and unkempt, embedded nails (cut, soaked paws in novalsan solution as patient would allow, some purulent d/c), moist dermatitis in perianal area from urine soaked hair covering area, general dermatitis (flaking and thick skin), epistaxis from jumping from transport vehicle (bleeding halted quickly), older animal; left alone in apartment for two weeks while owner was hospitalized
Eyes: bupthalmic appearance and opacity, possible glaucoma? matted d/c around OS
Ears: odorous ears but will not allow exam  Oral Exam: moderate to heavy staining, could not completely examine Heart: could not auscultate Lungs: could not auscultate Abdomen: WNL. Musculoskeletal: WNL BCS 5/9 Mentation: BARH  Preliminary Assessment: older animal, husbandry related medical issues. Plan: DVM intake
