Medical Behavior: Green; Sex: Female; Age: 10 years

DVM Intake Exam; Estimated age: 10 yo; Microchip noted on Intake? neg; History : O reportedly abandoned pet in apartment. Neighbor heard dog barking and brought dog in. Reports of bloody stool. Subjective: BAR, very sweet and friendly, no signs of aggression, good for PE, med handling, and x-rays. Observed Behavior – Objective T =; P = wnl; R = eup; BCS 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mod to severe dental disease ; PLN: submandibular LNs mildly enlarged; H/L: Grade V/VI left systolic HM with palpable thrill. NSR, CRT < 2, Lungs clear, eupnic; ABD: Severe bilaterally inguinal hernias, with soft, non-painful, non-reducible intestines/abdominal contents. No masses.; U/G: FI, no MGTs, no vulvar discharge.; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: attempted rectal but p would not allow; Assessment:- Severe bilateral inguinal hernias — r/o acquired vs congenital – Grade V/VI HM — congenital vs valvular disease vs endocarditis vs other cardiomyopathy – dental disease – reports of bloody stool; Prognosis: guarded; Plan: – ctm in medical until transfer/placement. Monitor hernias (strangulation, rupture), comfort, appetite, stools, any straining. – spoke with NH regarding case. Hernias need to be repaired but p has a severe HM. Recommend echo and surgery at a referral center with advanced monitoring capabilities. – Feed canned food only – Miralax 1/8th teaspoon in food BID. Adjust dose accordingly if diarrhea develops or p is straining to defecate – 2 view AXR — bilateral inguinal hernias with entrapped intestines. Lateral CXR – cardiomegaly
