Medical Summary Estimated age: 10-18 years old based on overall appearance Microchip noted on Intake? Scanned negative History : Found about a week ago, no medical hx given Subjective: Lies on table, has little interest in getting up. Observed Behavior – Relaxed, allows all handling. Objective BAR-H, MMs pink and moist, BCS 3/9, moderate muscle wasting EENT: Cataracts OU. Moderate black/brown waxy discharge AU. No nasal discharge. Oral Exam: Missing most teeth, tongue hangs out of mouth. Left maxillary PM 3(? Or 4, cannot tell) is still present, extremely mobile, and encrusted with a very large amount of calculus. Left max/mandib molars are also visible in the back of the mouth. PLN: No enlargements noted H/L: NSR, NMA! Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S. The tip of the penis occasionally protrudes, and there is a very small area of erythema on the mucosa. MSI: Can stand for a few seconds but prefers to lie down. No crepitus or joint swellings noted in any leg, no patellar luxation, no withdrawal on palpation. Moderate generalized muscle wasting on all legs. Skin of ventral abdomen and inguinal region is hyperpigmented and greasy. No fleas or flea dirt noted. No no masses noted. Fur coat is long and unkempt. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: 1. Severe dental disease 2. Otitis externa – R/O allergies vs. fur in ears and lack of grooming 3. Dermatitis, hyperpigmentation – R/O allergies vs. lack of grooming 4. Prefers not to stand – R/O arthritis vs. weakness vs. other Plan 1. Clean ears today and every 3 days, instill Otibiotic AU BID x10 days 2. Place third feeding sign 3. Recommend dental cleaning ASAP Prognosis: Fair SURGERY: Permanent ACC waiver due to AGE. 1088

Gerald was pretty weak when he arrived at the care center. He is such a valiant little guy whose stop by our facility might finally and rightly insured that he be loved and properly cared for in his golden years. Gerald is eager to be picked up from his kennel and carried tightly against my chest. He so enjoys the lap and would stay put for ever and ever. Believe it or not…he is a good little walker and has a ferocious appetite despite his missing teeth. He is minuscule, owns the cutest tiny face and lively eyes…and I have to be very careful not to step on him… Gerald so deserves a great and loving home…Come and meet him at once at the Manhattan Care Center.
