05/24/17 DVM Intake Exam Estimated age: 14 yrs Microchip noted on Intake? No. History : Stray dog, no medical concerns noted. Subjective: Observed Behavior – Friendly and social, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=88, RR=20. BCS=3/9. ORAL: Significant dental calculus, gingivitis, sensitive on oral exam. Missing multiple incisors. EENT: Nuclear sclerosis OU. No oculonasal discharge, no sneezing. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias, no coughing. ABD: Non painful, no masse/organomegaly palpated. U/G: Neutered male. MS: Ambulatory x 4, mod. bilateral hind limb weakness and stiff gait. Mod. stifle thickening and crepitus bilaterally, left worse. Mod. generalized muscle loss, especially prominent over spine, pelvis, and hind limbs. INTEG: Full, dry haircoat. No skin lesions or hair loss. CNS: Mentation appropriate. Mild CP deficits bilaterally in hind limbs. A: Geriatric dog with severe dental disease, hind limb arthritis, muscle loss and secondary muscle weakness. P: Scheduled for CBC/CHEM/T4. Feed soft food TID. Rx Rimadyl 100 mg 1 PO q 24 hours x 7 days. Needs dental cleaning with full mouth radiographs and extractions likely.
