Medical Summary 03/21/17 History – Stray dog, no medical concerns noted. No microchip found. Subjective Observed Behavior – Very friendly, social, playful. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. ORAL All adult teeth present. Slight calculus max canines. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: NSR, NMA. Lungs clear, eupneic. ABD: Non painful, no masses palpated. U/G: Intact female, mild vulvar enlargement with serosanguineous discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Lean body condition, BCS=4/9. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Apparently healthy young adult. Age est 1 year. P: OK for OVH surgery. Excellent prognosis.
