Did well for all medical handling and procedures. Objective ; P = wnl ; R = eupneic ; BCS 3/9; EENT: Eyes clear with nuclear sclerosis ou, ears clean, no nasal discharge noted; Oral Exam: adult dentition with severe dental disease, prognathism, and oligdontia, no oral lesions noted; PLN: No enlargements noted; H/L: NSR, Grade 4/6 HM, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles descended; MSI: Ambulatory x 4, skin free of parasites, no masses noted, dull hair coat with mild matts and erythematous ventrum; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric, Grade 4/6 HM, Dental disease, Plan: Continue to monitor while at BACC, Shaved matts off and trimmed nails, Rec echocardiogram for HM, Rec geriatric workup with placement, Scheduled medicated bath for tomorrow, Prognosis: Fair longterm prognosis, SURGERY: permanent waiver due to age and HM
