DVM Intake Exam: Estimated age: 8 yo Microchip noted on Intake? yesHistory : stray. Finder found dog yesterday, said p has not eaten, unsure if p has an injured jaw. Microchip was traced back to possible O and a voicemail left regarding reunification. Subjective: BAR. Overall, well behaved during PE and med handling. Would not allow thorough oral examination though, resisted and tried to bite. Observed Behavior -Objective: T = P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: pronounced mandibular prognathism, mandible deviated to the right, missing several teeth, moderate dental disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, no spay scar seen, no MGTs, no vulvar discharge MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: normal externally. Assessment: mandibular prognathism deviated mandible — r/o fracture dental disease intact female. Prognosis: good. Plan: – p was offered wet food during Intake. p was interested in eating, but could only lick up the food, as opposed to prehending it -monitor in medical overnight. offer dry and wet food and note how well p is able to eat – consider sedated oral exam, +/- xrays to rule out a fracture – p may need to be fed wet food for life, or placed with an adopter willing to hand feed. SURGERY: Okay for surgery
