Medical Behavior Evaluation BLUE
Medical Summary Brought in as a stray, no medical concerns noted. Microchip scan neg. Calm, quiet, allows all handling. BARH. mm=pink, moist, CRT<2 s. P = 80 R = 20 BCS = 4/9. ORAL: Mild calculus, gingivitis. Area of discolored or necrotic gingiva above L max C. No swelling or discharge. EENT: OU – mild chemosis, crusted material, scleral injection. No nasal discharge. Ears dirty AU, no swelling or erythema. PLN: No enlargements noted. H/L: NSR, NMA. Lungs clear, eupneic. ABD: Non painful, no masses palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lameness. INTEG: Full, dirty haircoat. No skin lesions. Nails overgrown. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Age est. 5 yrs. Dental disease with possible abscess or oronasal fistula above L max C. Mild conjunctivitis. P: Cleaned ears, trimmed nails. Cleaned eyes with saline. Start TAB-H ointment BID x 7 days. OK for neuter surgery. Good prognosis.
