DVM Intake Exam; Estimated age: 10; Microchip noted on Intake? ; History : Transported from Brooklyn ACC ; Subjective: QARH; Observed Behavior – Nervous, tense, timid ; Objective ; T = – P = 100; R = wnl; BCS = 4/9; EENT: OU mucoid dis-charge, ears clean, no nasal or ocular discharge noted – lenticular sclerosis; Oral Exam: Teeth in good cond – minor calculus C1 on pre-m and molars; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: INTACT MALE; MSI:  Marked Paresis on the hind-limbs ; – PT is unable to ambulate fully on the hind end, very stiff  – RH abduction with delayed placement, normal withdrawal – allowed for flexion and extension. Normal drawer sign – LH normal placement, normal withdrawal, allowed for flexion and extension. Normal drawer sign – Palpation of the dorsal spine revealed no obvious pain response ; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: externally normal ; Assessment- Marked paresis of the hindlimbs with delayed reflexes and moderate abduction of the hind right leg when resting ; Plan:Sedated xrays – V/D and Lateral Thoraco-Lumbar Spine – V/D and Lateral Pelvis – V/D and Lateral R and L Hind legs ; Sx; Permanent waiver due to age
