Medical Summary microchip scan positive 985121003125514 female; approximately 10 yrs old BARH eyes appear cloudy moderate derbis in ears no nasal discharge dental disease ulcerated mammary tumor on lower right ventrum foul smelling odor dermatitis on thighs and tail feces stuck to tail causing irriation; area was shaved nails were overgrown and curling; gave nail trim 3.8lbs; BCS: 3/5 has a good appetite, ate A/D food well calm, allowed all handling medication as per OC dr. 1173: 0.1ml buprenorphine 0.3mg/ml SC BID x 4 days 0.6ml amoxi clav potas. 45.7mg/ml BID x 10 days will need to be transported to BACC or MACC either tonight or tomorrow to be seen by a vet vet exam scheduled for tomorrow
