Medical Summary DVM Intake Exam Estimated age: 8 years Microchip noted on Intake? scan negative History : owner surrender; history of chronic skin condition, discharge from eyes, and bloody diarrhea if he eats dog food Subjective: Observed Behavior – tolerated exam well Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 4/9 EENT: OU- corneal opacity, vascularization, green mucoid dicharge, conjunctival hyperemia, normal to delayed menace response; AU- erythema, swelling, tan debris; no nasal discharge noted Oral Exam: mild/moderate dental calc PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, 2 descended testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: wnl, no blood on glove Assessment severe dermatitis- R/O atopy vs food allergy vs mites vs other otitis externa keratoconjunctivitis- R/O KC vs other Plan STT: 0 OU skin scrape: negative cleaned ears and applied osnuria, repeat in 1 week start simplicef 50 mg PO SID x 4 weeks start prednisone 5 mg PO BID x 5 days, then 5 mg PO SID x 5 days, then 5 mg PO EOD x 5 doses feed z/d diet only Prognosis: fair SURGERY: Temporary waiver due to dermatitis
