DVM Intake Exam; Estimated age:10y; Microchip noted on Intake?yMicrochip Number (If Applicable):985112002746587; History 😮 surrender; Subjective: iatrogenic wound to nose when trying to muzzle–began to bleed transiently. Observed Behavior -snapping, trying to bite, could not muzzle. sedated 0.4 dexdomitor IM–still required muzzling and extreme restraint. Reversed with antisedan 0.4ml IM after exam; Objective ; T =; P =60; R =wnl; BCS 7/9; EENT: Eyes clear, AS large scab with black discharge and blood beneath it–once cleaned no masses seen although otoscope not used (too much moist dc and dog thrashing)–apparently otitis was walled off by matt, no nasal or ocular discharge noted; Oral Exam:severe dental disease ++++ PLN: No enlargements noted; H/L: NSR, NMA, , Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G:neutered male; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Severe otitis AS, severe dental disease; Prognosis:fair; Plan:applied Claro ointment to left ear–other treatments will not be practical. Recc dental after geriatric workup in future; clavamox 62.5 mg bid x 14 d for ear and teeth
