Medical Summary Came into the bronx shelter — underweight / severe matted / geriatric / severe congestion — struggling to breath / matts surrouding face and eyes — unable to see — dehydrated / severe tartar and possible dental disease Microchip: negative Sex: male Age 12 Y Geriatric Mentation: BA Eyes: unable to see due to matts (wont allow you to get close to face / or touch face — attempt to bite) Ears: matted Nose: congested Teeth: missing most of his incisors / severe tartar / possible dental disease If abnormal BCS: 2 — severe underweight Skin: matted Hair Coat: fleas negative Any injuries: physically none but does have other medical condtions occuring that the VC will determine Behavior: tense / tried to bite / doesnt really like to be touched Medication: VC will determine NOSF
