DVM Intake Exam; Estimated age: 9 years; History : owner surrender; Subjective: Observed Behavior – friendly but tense during exam; Objective ; BCS 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: moderate dental calc; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: F, no spay scar seen; MSI: Ambulatory x 4, skin free of parasites, no masses noted, a few epidermal collarettes at ventral abdomen; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; mild dermatitis; moderate dental calc; underweight- R/O dietary vs GI disease vs renal disease vs endocrine disease vs other; Prognosis: fair; Plan: start simplicef 25 mg PO SID x 2 weeks; SURGERY: Okay for surgery ; recommend bloodwork prior to surgery
