Medical Summary DVM Intake Exam Findings History – Stray dog. Approx 5 yrs, M neutered. Scan positive #985121012980210. Subjective Observed Behavior – Shy, calm, allows all handling. Objective: BARH. mm=pink, moist, CRT<2 s. P = 100 R = pant BCS = 5/9 ORAL: Mild calculus, gingivitis. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted H/L: NSR, NMA. Lungs clear, eupneic. ABD: Non painful, no masses palpated. U/G: Neutered male. MSI: Ambulatory x 4, no lameness. Skin free of parasites, no masses noted, clean well-groomed hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities. A: Mild dental disease, otherwise apparently healthy adult dog. P: OK for vaccines. Excellent prognosis.
