Medical Behavior Evaluation GREEN
Medical Summary MC scan – positive. BCS 6/9.EENT: Eyes clear. AU pinnae are lichenified with erythema, some hyperpigmentation, and mild brown discharge. Moderate serous nasal discharge. Oral Exam: Moderate tartar especially on canine teeth. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated. U/G: Male intact. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Pt has dewclaws (1st digits) on hind feet – the toenails on these toes are very long and curled around – pushing into pad of toe on the left hind foot. CNS: Mentation appropriate – no signs of neurologic abnormalities. Assessment ; Chronic otitis externa, Dental disease. Prognosis: Good. Okay for in-house surgery. Plan: Cleaned ears and applied Otibiotic during exam, continue Otibiotic BID x10 days and ear cleanings q3 days x3 additional cleanings. Recommend dental cleaning

Basic information:
Floppy is a 9 yr. old Beagle Chihuahua mix. The Owner got her from a friend as a gift; she has owned Floppy for over 5 yrs. The owner surrendered Floppy because her landlord no longer allows pets. Floppy has no health problems and hasn’t been taken to a vet in a couple of days.
Socialization:
Around strangers Floppy is friendly playful. Floppy has lived with children between the ages of 6 and 13 years old, he is playful around children and adults he does not know. Floppy has lived with another small dog and he was relaxed around the other dog and does not bark at dogs outside. Floppy has never bitten anyone.
Behavior:
The owner stats that Floppy is not bothered when some moves his food or treats while he is eating, when someone takes his toys away or when someone disturbs him while sleeping. Floppy likes to be brushed and given bathes but has never had his nails trimmed. When an unfamiliar person or animal approaches a family member or his home he approaches them in a friendly manor. The owner says Floppy is nervous around loud noises such as storms or fireworks and may try to hide from the noise.
For a new family to know:
The owner describes Floppy as friendly, affectionate and playful with a medium activity level. His favorite activity is to play fetch with his owner. In the home Floppy tends to follow the owner around the house and is kept mainly indoors. Floppy sleeps in the floor of the bedroom on a blanket. Floppy was fed professional + dry dog food twice a day. He is house trained and has never had an accident in the home, when outside he goes to the bathroom on or around trees and grass. Floppy knows the commands “sit “and “down” when spoken in Spanish. For exercise Floppy is used to brisk walks on the leash and twice a day and does not pull on the leash unless he is excited.

ASSESSMENT:
Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Dog trembles, and was had a tense body posture.
Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, his tail is between his legs, body tense, and mouth closed, ears back.
Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, He is focused on stimuli other than the Assessor.
Squeeze 1: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are back.
Squeeze 2: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are back.
Toy 1: No interest.
Summary: Floppy was a bit tense, but allowed all handling during his handling assessment.

According to Floppy’s previous owner, Floppy has lived with another small dog and he was relaxed around the other dog and does not bark at dogs outside. When off leash with dogs at the Care Center, Floppy is social, checking in on the other small male and female dogs as he slowly explores the pen.
MEDICAL BEHAVIOR:
During his initial medical examination, Floppy allowed all handling.
ENERGY LEVEL:
Floppy has displayed a low energy level during his stay at the care centers.
