DVM Intake Exam; Sedated with 0.2 ml dexdomitor IM for hindlimb radiographs; LH-complete luxation of coxofemoral joint, incomplete fracture of distal fibula; congenital bowing of femur; RH-hairline fracture of distal fibula; congenital bowing of femur; A; 1. L coxofemoral luxation, suspect traumatic; 2. Bilateral distal fibular fractures; P; -hydromorphone 0.25 ml IM tonight and twice tomorrow then try to switch to gabapentin; -carprofen injection 50 mg/ml: 0.5 ml SQ; continue orally tomorrow; -recommend RHO after placement and 6 weeks of post-operative activity restriction and rehabilitation (underwater treadmill, strengthening exercises, massage, etc); overall prognosis good with surgery
Vet Notes: 9:46 AM; Estimated age: approx 1 year; History: finder saw dog get hit by a car; Subjective: QAR; Observed Behavior -docile, allows handling; Evidence of Trauma seen -non weight bearing LH, reportedly HBC; Objective ; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mm pk, tacky; CRT 2 sec; adult dentitia with no significant tartar, staining; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: female; MSI: ; -ambulatory x 3 with non weight bearing LH lameness, moderate soft tissue swelling along thigh; -slightly overgrown hair coat but healthy; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal; Assessment; 1. Possible HBC; 2. LH lameness; Prognosis: fair to good; Plan: -gave 0.25 ml hydromorphone SQ at 9:45 am; -recommend sedated pelvic, LH radiographs; -carprofen 50 mg/ml: 0.5 ml once today, then continue 25 mg PO SID
