Medical Summary Scan : neg Sex: MI QAR moderate dehydration, tries to bite when examined sedated with 0.01 cc hydromophone; 0.05 cc dexdormitor; 0.05 cc telazol cc IM EENT: OD corneal ulceration with hyperpigmentation, severe periocular discharge and swelling ; severe dental disease with multiple teeth falling out on physical exam CV/L 2/6 L systolic murmur under sedation INT: thick yellow crusting extending deep through dermis on L chin, OD periocular, perianal, prepuce/prescrotal and R flank area. ABD: firm material in caudal abdomen MS amb x 4 BCS: 1/9 severely emaciated LN wnl palpated firm material in caudal abdomen Lateral radiograph showed radiopaque fragment in colon HWT neg O: A: 1) OD corneal ulcer 2) diffuse pyoderma 3) emaciation 4) phimosis 5) mild heart murmur under anesthesia 6) foreign material in colon (likely non issue 7) severe denta disease P: 0.25 cc convenia SC, cleaned wounds with saline and applied SSD cream to body lesions and neopolybac to periocular and ocular lesion. Gave 100 cc SC LRS. If doing well, repeat convenia in 2 weeksPROGNOSIS: FAIR
