Medical Behavior Evaluation YELLOW
Medical Summary DVM Intake Exam Estimated age: 12-14 years Microchip noted on Intake? placed on intake History : brought in as stray Subjective: BAR Observed Behavior – tries to bite, yelping during exam; blind on L side . Objective BCS 3/9 EENT: OS-mild microophthalmia/enophthalmia; sevre corneal edema; decreased-absent menace OD-immature cataract no nasal discharge ears clean Oral Exam: mm pk, sl tacky; CRT <2 sec; severe tartar build up PLN: No enlargements noted H/L: poss grade II/VI murmur; eupnic, normal lung sounds in all fields ABD: Non painful, no masses palpated U/G: female intact; 2 mammary gland tumors-one approx the size of a marble in caudal gland and one slightly larger tumor in middle L gland; no ulceration MSI: bilateral hind limb lameness with bowing of stifle joints with moderate chronic thickening; grade IV MPL on LH and grade II on RH mild irritation under tail/perivulvular; small (<0.5 cm) round, sl raised, erythematous lump near vulva-suspect chronic inflammation CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Geriatric Blind/corneal edema OS-r/o chronic uveitis vs glaucoma vs other Mammary Gland Tumor Poss low grade heart murmur Bilateral MPL with arthritis and lameness Dental Disease Plan Rec’d NH placement Rec’d biopsy of MGT-incisional vs excisional Rec’d ophthalmology consult ideally; at minimum, need to take IOP to r/o uveitis vs glaucoma (unable to perform due to lack of working tonopen) Rec’d surgical consult to discuss benefits of pursuing surgical correction of severe MPL; due to age, chronic pain management +/- rehabilitation may be alternative to surgery Rec’d dental cleaning with multiple extractions likely Prognosis: guarded to fair; prognosis very dependent on histopath results from MGT SURGERY: perm waiver due to age
