History : owner surrender due to owner’s health problems Subjective: QAR Observed Behavior – timid/shy but docile and allows all handling Objective BCS 6/9 EENT: OU-immature cataracts, mild tear staining along face; n nasal discharge, ears clean Oral Exam: mm pk, moist; CRT <2 sec; moderate tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: tense but nonpainful, no palpable abnormalities U/G: female; no spay scar or tattoo noted MSI: Ambulatory x 4, skin free of parasites, no masses noted, mild dandruff, flea debris along dorsum; several small sebaceous adenomas-one in between eyes, one on top of head, one on RH digit and one on medial aspect of distal LH CNS: mentation appropriate – no signs of neurologic abnormalities Rectal:not performed Assessment Dental disease Geriatric Cataracts Several sebaceous adenomas Plan Rec’d geriatric blood work and dental cleaning after placement Prognosis: good SURGERY: permanent waiver due to age

Look: 2. Dog’s eyes are averted. Her body posture is stiff and fearful, her tail is low and not moving. She allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 2. Dog stands still and accepts the touch, her eyes are averted, her tail is between her legs, body stiff, mouth closed.

8/29: When introduced off leash to other dogs, Evie keeps mostly to herself.
MEDICAL BEHAVIOR:
During her initial medical exam, Evie was shy but allowed handling.
ENERGY LEVEL:
We have no history on Evie so we cannot be certain of his behavior in a home environment.
RECOMMENDATIONS: Experienced adopter, adult-only home (suitable for an adopter with some previous dog experience, especially with behaviors outlined below)
_X_ No children (under 13): Due to how uncomfortable Evie is currently with touch and novel stimuli, as well as her owner stating she is fearful of strangers, we feel that an adult-only home would be most beneficial at this time.
Potential challenges:
_X_ Fearful: Evie is fearful at the care center. It is important to always go slow and give Evie the option to walk away from any social interaction. Evie should never be forced to approach anything that she is uncomfortable with or to submit to petting or handling. It should always be Evie’s choice to approach a new person or thing. Evie would do best in an initially calm and quiet home environment and should be given time to acclimate to her new surroundings.
