Medical Summary Microchip: 432344721C Sex: intact male Age: appx 12-14y Mentation: QARH Eyes: KCS appearance, occ. nystagmus Ears: clean Nose: no d/c but heavy scaling Teeth: severe ddz If abnormal BCS: WNL Skin: WNL Hair Coat: heavily matted and covered in outdoor debris/brambles, shaved some but dog was intolerable; possible tick infestation seen in mats (crawling extoparasites) but dog would not allow closer inspection Declawed: N/A Any injuries: reported “found by roadside”, dog was lame and unwilling to move, ataxic when walking (possibly from mats affecting gait); cries out in pain when front limbs are manipulated, could not see any injury; laceration to top of head, clipped, appx 1in, not actively bleeding Behavior: quiet at first but when stimulated tries to bite and struggles in hold Medication: N/A, preventatives given, sign up for sedated shave
