Intake Date: 5/29/18  Intake Type: Owner Surrender

Medical Behavior: Green  Sex: SpayedFemale  Age: 11 years

Weight: 64 lbs

DVM Intake Exam : Estimated age: 12  Microchip noted on Intake? n  Microchip Number (If Applicable): n  History: Owner brought patient in to another ACC location requesting medical care for this patient. Apparently after conversation with ACC staff the owner requested for the patient to be euthanised. The pt was then transferred to MACC  According to notes, patient has a history of seizures.  Subjective: BARH  Observed Behavior – gentle  Evidence of Cruelty seen – no  Evidence of Trauma seen -no
Objective : T = –   P =wnl  R =wnl  BCS= 6.5/9  EENT: Eyes – lenticular sclerosis, ears brown greasy debris no nasal or ocular discharge noted  Oral Exam: Calculus on canines and premolars  PLN: Right SM lymph node enlarged (2cm x 2cm)   H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic  ABD: SQ mass on cranial abdomen – suspect lipoma/fat pads R/O MGT, Non painful, no masses palpated  U/G: SPAYED – tatto present  MSI:  Overgrown nails embedded in foot pads – nail beds purulent discharge Paws/Digits/Interdigital space – erythema, dermatitis, inflammation  Ambulatory x 4 but uncomfortable when walkijng, skin free of parasites, no masses noted, healthy hair coat  CNS: Mentation appropriate – no signs of neurologic abnormalities  Rectal: externally normal Assessment : 1) Skin on Paws/Digits/Interdigital space – Marked erythema, dermatitis, inflammation, pyoderma  2) Right SM lymph node enlarged  3) Apparent seizure history according to owners   Prognosis: Good Plan:
+ Rimadyl 2.2 mg/kg SID for 5 days  + Simplicef 4.5 mg/lb SID for 21 days (300mg)  + Soak feet in dilute chlorohexidine solution and dry Q 3 days for 14 days + Clip nails

A Little Bit About Me…

A volunteer writes: Epiphany is hoping that any minute now you’ll have a revelation and realize she’s the pet for you. I mean, c’mon, just look at that sweet face! Age is no barrier to love and though she’s 11 years old, our frosted beauty still has plenty of personality and even a little playfulness tucked away for the happy day she leaves for her new home. Epiphany welcomes visitors with tail wags and she’s never anything but soft and affectionate, greeting people and pups politely and allowing all handling fuss-free. She’s slow moving on leash as her paws are still healing but can take the stairs up and down with ease and in playgroup she’ll even bounce around with the younger dogs, though she’s not above scolding them if they get too rough! Epiphany earned a stellar behavior rating, seems quite house trained and is still very much ‘with it upstairs’ (as my Mom would say) and would be such an easy pet to welcome into your life. She’s making the best of things here at the Care Center but in her heart Epiphany knows that a forever home is the real golden ticket to living out her dreams of a joyful, comfortable retirement. Date of assessment: 3-Jun-2018

Look: 1. Dog’s eyes are averted, with tail wagging and ears back. Allows head to be held loosely in Assessor’s cupped hands.

Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.

Tag: 1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.

Flank squeeze 1: 1. Dog does not respond at all.

Toy: 1. No interest.

Summary: Epiphany was affectionate towards the assessor, allowing all handling without issue.

PLAY GROUP :

6/3: When introduced off leash to the male greeter dog, Epiphany engages in brief bouts of play. She offers correction when play becomes more exuberant.

ENERGY LEVEL: Epiphany displays a low energy level in the care center.

BEHAVIOR DETERMINATION: AVERAGE (suitable for an adopter with an average amount of dog experience)
