Apparently after conversation with ACC staff the owner requested for the patient to be euthanised. The pt was then transferred to MACC According to notes, patient has a history of seizures. Subjective: BARH Observed Behavior – gentle Evidence of Cruelty seen – no Evidence of Trauma seen -no
Objective : T = – P =wnl R =wnl BCS= 6.5/9 EENT: Eyes – lenticular sclerosis, ears brown greasy debris no nasal or ocular discharge noted Oral Exam: Calculus on canines and premolars PLN: Right SM lymph node enlarged (2cm x 2cm) H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: SQ mass on cranial abdomen – suspect lipoma/fat pads R/O MGT, Non painful, no masses palpated U/G: SPAYED – tatto present MSI: Overgrown nails embedded in foot pads – nail beds purulent discharge Paws/Digits/Interdigital space – erythema, dermatitis, inflammation Ambulatory x 4 but uncomfortable when walkijng, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: externally normal Assessment : 1) Skin on Paws/Digits/Interdigital space – Marked erythema, dermatitis, inflammation, pyoderma 2) Right SM lymph node enlarged 3) Apparent seizure history according to owners Prognosis: Good Plan:
+ Rimadyl 2.2 mg/kg SID for 5 days + Simplicef 4.5 mg/lb SID for 21 days (300mg) + Soak feet in dilute chlorohexidine solution and dry Q 3 days for 14 days + Clip nails

Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.
