DVM Intake Exam Estimated age: 2-5 years based on dentition and secondary sex characteristics Microchip noted on Intake? Scanned neg on intake History : Found tied up outside Subjective: VERY fearful – crouched, tail tucked, moves slowly. Actively lunges and snaps at handler. Mouth is bleeding. Emaciated. Evidence of Cruelty seen – Severely emaciated Evidence of Trauma seen – Bloody fluid coming from mouth Sedated for exam – could not examine without sedation due to pt lunging at examiner. Gave 0.8 ml hydromorphone and 0.2 ml Dexdomitor IM. Waited 45 minutes – pt still lunging and growling. Administered 0.3 ml Telazol IM. Waited 55 minutes – pt still snapping. Administered 0.2 ml Telazol IM. Waited 30 minutes – pt fully sedate and we were able to complete the exam. Objective Sedated, MMs pink and moist, BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Right maxillary and mandibular canine teeth are heavily worn down. Left maxillary and mandibular canine teeth are recently fractured, with sharp edges and visible pulp exposure. A large amount of the left cheek is swollen with several lacerations. Bloody saliva dripping from mouth. No active bleeding. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Tucked up, non painful, no masses palpated U/G: Female, small nipples, developed vulva MSI: Ambulatory x 4, emaciated. Skin free of parasites, no masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. Emaciated 2. Fractured teeth – left maxillary and mandibular canines were fractured when pt bit down on metal catch pole this morning, but the right canines were previously damaged Plan 1. Feed three times daily 2. Rimadyl for oral pain and inflammation – 50 mg PO SID x10 days 3. Recommend dental cleaning and extractions (vs. root canal vs. other treatments for fractured teeth) Prognosis: Good SURGERY: Temporary waiver due to EMACIATION. 1088

KNOWN HISTORY: Limited Profile Date of intake 8/28/2017 Unaltered female, stray Behavior toward children: Finder brought Emily Rose inside and she snapped at Finder’s daughter SAFER ASSESSMENT: Date of assessment 8/30/2017 Summary: Emily Rose has been difficult to remove from her kennel, extremely fearful since intake. She has allowed minimal handling. Out of concern for her stress levels we feel she is not a greta candidate for a handlign assessment at this time. INTAKE BEHAVIOR: Date of intake 8/28/2017 During intake, Emily Rose was very fearful, growled and snapped. MEDICAL BEHAVIOR: Date of initial 8/28/2017 During her initial medical exam, Emily Rose was very fearful, crouched, tail tucked. She lunged and snapped at handler. ENERGY LEVEL: We have no history on Emily Rose so we cannot be certain of her behavior in a home environment. In the care center, she is fearful and moves slowly, low energy. RECOMMENDATIONS: New Hope _X_Place with a New Hope partner – Emily Rose has not acclimated well to the kennel environment and has allowed only minimal handling since intake. We recommend placement with a New Hope partner who can provide any necessary behavior modification (force-free, positive reinforcement based) and re-evaluate behavior in a stable home environment before placement into a permanent home. Potential challenges: _X_Fearful/potential for defensive aggression – Emily Rose has attempted to bite during her stay in our care centers when uncomfortable; guidance from a professional trainer/behaviorist is recommended to assess behavior after decompression in a new home environment. Force-free, reward based training is advised when introducing or exposing Emily Rose to new and unfamiliar situations.
