Medical Summary DVM Intake Exam Estimated age: 5-8 Microchip noted on Intake? n History : stray Subjective: Observed Behavior – nervous, allows exam Objective P = 100 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mod tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: mammary tumor in left caudal MG, 1″ daimeter, freely movable, irregular MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: MGT Plan: needs excisonal biopsy , chest radiographs Prognosis: fair to guarded: MGT are 50/50 malignant vs benign–work up is the only way to tell the difference SURGERY: Permanent waiver due to tumor
