DVM Intake Exam; Estimated age: ~12-14 years; Microchip noted on Intake? negative; History : stray, not eating. Underweight; Subjective: QARH. Regurgitated once during exam; Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures.; Objective ; P = wnl ; R = eupneic ; BCS 2/9; EENT: Eyes have nuclear sclerosis ou with no d/c, ears clean, no nasal discharge noted; Oral Exam: severe dental disease and fractured canines; PLN: No enlargements noted; H/L: NSR, Graed 3/6 HM, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles descended; MSI: Ambulatory x 4, live fleas, no masses noted, unkempt hair coat, underweight. Both front legs shaved and square area on top of his head with a small ~0.25cm wound with no swelling or d/c; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric-suspect underlying condition; Grade 3/6 HM; Underweight; Severe dental disease; Small wound on top of head; Mild hyponatremia and hypochloridemia r/o secondary to losses such as vomiting or diarrhea; Mild non-regenerative anemia r/o blood loss vs neoplasia vs other; Elevated LEs; Plan: Continue to monitor while at BACC; Start cerenia 1mg/kg SQ SID x5d until 7/27 (first dose given)-did not give SQ LRS due to HM and well hydrated; CTM in VS +/- AXR if no improvement. Rec NH placement for AUS. CBC/Chem/T4 pending, Prognosis: Fair to guarded, SURGERY: permanent waiver due to age
