DVM Intake Exam:Estimated age: 3 History :  Subjective: barh  Observed Behavior – fractious. Objective:  T = – P = wnl R = wnl  BCS = 5/9 EENT: Eyes clear, ears erythema and brown.yellow debris, no nasal or ocular discharge noted Oral Exam: teeth in good cond PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful,  no masses palpated U/G: Two descended testicles  MSI: Matting around legs and abdomen, Ambulatory x 4, skin free of parasites, no masses noted,  CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal:externally normal. Assessment:-otitis externa -matted -otherwise healthy  Prognosis: good   Plan:  neuter – cleaned ears and applied claro otic on intake  SURGERY: Okay for surgery   Recommend grooming
