Intake condition UNH&UNTREA Intake Date 03/21/2017, From NY 11237, DueOut Date 03/21/2017,

DVM Intake Exam Findings; scan negative; History    owner surrender due to cryptorchid and destructive behavior; Subjective     appears sick; Observed Behavior –  docile; Evidence of Cruelty seen –  neglect, pet very pale, cirtically ill; Evidence of Trauma seen –  none; Objective  T =    99.3          P =  136     R =  44    BCS 5/9; EENT:  depressed Eyes clear, ears clean, no nasal discharge noted; Oral Exam:   pale and slightly icteric; PLN:  No enlargements noted; H/L:  NSR, III/VIbilat HM,   Lungs clear, eupnic; ABD:  Non painful, no masses palpated; U/G:   one scrotal testeswichi is small, one enlarged SC mass at inguinal region approx 8 cm diam; MSI:  Ambulatory x 4, skin free of parasites, pale in pallor, hypotrichosis generalized; CNS:  mentation appropriate – no signs of neurologic abnormalities; Rectal:  firm dried out fecal balls with hairs; Assessment critically ill, r/o  sertoli cell tumor; Medical Behavior Status blue; Medical Status UNH $ UNTREATABLE; Asilomar Status 5NC; Fast Track no critcally ill; Plan; no vaccines, dewormer or activyl applied; cbc NEUT 1,9700 bands suspected; HCT 8.2%; PLT 13,000; chem WNL; T4 WNL; hwt negative; parvo snap; NH vs HER

PROFILE:

Drizzy is a 6 year old male white and brown Shih Tzu mix; he came to the family from a pet shop, and is being surrendered due to Landlord issues. Drizzy has a possible cryptorchid, but no other health issues or injures. He has not been to vet recently.

Around strangers Drizzy is friendly and outgoing but could be mouthy when he is playing with a toy.  He has been around children of ages 8 month old and 3 years old, and relaxed and playful. When Drizzy sees another dog he because tense and stay away from the dog.  Drizzy has no bite history.

Drizzy will growl if someone touches his food bowl, and isn’t bothered when someone tries to pick him up he isn’t bothered. During Bathing Drizzy is very tense and tries to run away. When an unfamiliar person approaches the home he barks at first but becomes friendly.

He was described as friendly, affectionate and play by his previous family with a high activity level. Drizzy loves playing with stuffed toys, only stays indoors. He is feed dry and wet food 3 times a day. He is house-trained and goes on the wee-wee pad.  When home alone he will scratch on the walls and chew any wood furniture.  He has never been crate trained, and has never been off-leash.
