Medical Summary DVM Intake Exam Estimated age: 10 years Microchip noted on Intake? neg, placed on intake History : owner surrender; previous owner did not have him very long; history of intermittent blood in stool Subjective: BAR Observed Behavior – easy going, allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted; small amnt of dark crusting discharge on top of pinna AD-history of being nicked with clippers Oral Exam: mm pk/pigmented, moist; severe dental disease; underbite PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: RH-grade I lameness with wide based stance in hind limbs; chronic thickening of stifle with no discomfort on extension, mild drawer; no significant muscle atrophy compared to LH nails overgrown CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Geriatric Dental disease Lameness Plan Rec’d dental cleaning with likely multiple extractions Prognosis: good; suspect changes to R stifle are indicative of a chronic injury to the CCL with secondary arthritic changes; there is no significant discomfort at this time but chronic pain management may be indicated in the future as arthritis progresses SURGERY: neutered
