DVM Intake Exam; Estimated age: 5 years ; Microchip noted on Intake? scan negative; History : stray; was transported from Greater Staten Island Veterinary Services; she had a nail digging into her paw pad at her left hind; her nails were clipped; Subjective: Observed Behavior – very tense but tolerated exam; Objective ; BCS 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: limited oral exam; moderate dental calc; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: Female, no spay scar seen; MSI: Ambulatory x 4, skin free of parasites, swelling and erythema at outer digit of left hind paw; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; swelling at LH digit- R/O infection/inflammaton from ingrown nail vs neoplasia vs other; Prognosis: good; Plan:; start clavamox 250 mg PO BID x 7 days; SURGERY: Okay for surgery
