06/22/17 14:20 Basic Info: Dolla A1115000 is a 3yr old male gray & white short haired mix who was purchased off of someone selling puppies at a month old & is being surrendered due to too many pets. He hasn’t been seen by a vet since 2016 & currently has some skin allergies. Socialization: Dolla is shy for several minutes with strangers. He was gentle, exuberant, relaxed & playful with the adults and 2yr & 3yr old children in the home. Dolla hasn’t bitten/scratched any person or animal. He lived with 5 dogs & 2 neutered cats for about 8mths (cats) and was relaxed, playful and very rough at play with them at times. Behavior: Dolla isn’t bothered by the following: having his food/food bowl, toys/treats touched/moved, being moved off of furniture or being brushed. He hasn’t been disturbed while sleeping/resting or had his nails trimmed. Dolla struggles to get away during baths. If someone unfamiliar approaches the home/family member he’ll bark, back away some then approach to greet. For A New Family to Know: Dolla is described as playful, excitable & affectionate with a medium-high activity level. When the owner is home Dolla follows them around. Dolla enjoys ball, stuffed, rope toys & games of tug & chase. He sleeps in a dog crate, eats Beneful dry food 2x a day & is housebroken. Dolla is an indoor mostly dog that is let out to the yard 3-4x a day and his known commands are “sit” & “go home” (to go into his crate) that he doesn’t always listen to. Dolla is walked around the neighborhood 2x a day where he pulls on his leash at times. Intake Behavior: Dolla had a loose, wiggly body (especially after I gave him some treats) & allowed handling. He allowed to be scanned (-), collared & taken pictures of. Dolla walked alongside us (his owner & I) to his kennel where the owner told him to “go home” and he entered the kennel on his own after a couple of tries.

A volunteer writes: Dolla for dolla, you won’t find a more loving and fabulous dog than our sweet Dolla. Part of his human family for his entire life, he was surrendered as there are currently too many pets in the home. His medical intake note states: “SUPER calm and SUPER friendly!! Pet leans into me when I scratch his cheeks and ears”, and I couldn’t agree more. Snuggly, soft and gently wiggly, Dolla loves putting his head against my chest for cuddling, loves to be petted, and is simply the best cuddler on four legs. It was hot and humid, and Dolla’s tongue was hanging out, but that didn’t stop him from enjoying a good cuddle. Dolla lived with a 2 and 3 yo child and was gentle, relaxed and playful with everyone in the home. He also lived with 5 dogs and 2 cats and was also relaxed and playful, though sometimes rough at play. Dolla slept in his crate (“go home”), isn’t fond of baths, is housetrained, doesn’t guard his stuff, loves treats, and enjoys playing with toys, tug and chase being favorites. I adored my time with this snuggle meister. He has no match in the sweetest of cuddles department. He’s great on leash, takes treats nicely, notices other dogs with interest but no pulling, and hopped right back into his kennel when our time together was over. He needs TLC for his coat, as his former owner switched his food brand, which he may be allergic to. Dolla will make you feel that there is no one on the planet that he’d rather spend time with, and I love that about him. So you will! Don’t pass up the chance to meet our snuggle boy today.

DVM Intake Exam Estimated age: Reported to be 3 or 4 years old. Exam is consistent with this. Microchip noted on Intake? Scanned negative. History : Owner surrender. Client used to feed Taste of the Wild food with a Nupro supplement. A month or so ago, he switched to feeding Beneful with no supplement. Dolla lost a lot of weight and a lot of his hair fell out, and he developed lots of itchy sores. Dolla also used to chew the tip of his tail a long time ago. In the last few weeks he started chewing it again and it’s a little swollen now. He has not had any diarrhea, but he has been vomiting recently. Last vaccines were when he was a puppy. Subjective: SUPER calm and SUPER friendly!! Pt leans into me when I scratch his cheeks and ears. Very relaxed body. Pt allows all handling, including vaccination, MC placement and blood draw without a care. Objective BCS 3/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Soft, tucked up, non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Generalized patchy alopecia with erythematous skin and crusty lesions. Interdigital dermatitis x4 feet. The tip of the tail is swollen, blunt-ended and irregular, as if the very tip was chewed off at some point. Ambulatory x 4, skin free of parasites, no masses noted CNS: mentation appropriate – no signs of neurologic abnormalities Assessment 1. Dermatitis, alopecia – history and clinical signs are consistent with food allergy (to something in the Beneful food), although an environmental allergy is also possible 2. Underweight – likely secondary to food intolerance 3. Dental disease Plan 1. Cephalexin 750 mg PO BID x21 days – may be extended if necessary 2. Benadryl 75 mg PO BID x14 days – may be continued indefinitely if helpful (or switched to another antihistamine) 3. Place third feeding sign 4. Recommend dental cleaning in the near future 5. If a diet change doesn’t fully fix pt’s dermatitis, then a hypoallergenic diet trial should be started Prognosis: Good with appropriate diet/allergy management SURGERY: Temporary waiver due to WEIGHT and DERMATITIS 1088
