Initial Exam (3/09/17);DVM Intake Exam Findings; Observed Behavior – calm; Evidence of Cruelty seen – n; Evidence of Trauma seen – yes; Objective; BCS 5/9; bloodwork: alt974 ggt 15 unremarkable otherwise; EENT: OD bupthalmic, partially proptosed–did not notice at first but became more pronounced after blood draw. clearly chronic–injected sclera, dry eye; Oral Exam: mod tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: neutered; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment; proptosed eye, poss history seizures, elevated liver enzymes; Plan:; cbc/chem run, needs bile acid profile and ultrasound; recc nh placement; tab ou for eye, may need enucleation
PROFILE:
Divo is friendly and outgoing towards strangers. He lives with his owner’s daughter who is 10 years old and he is relaxed and playful with her. He plays gently with both adults and children. He is relaxed and playful with all the dogs he meets outside and loves to approach them to play. He plays more exuberantly with them. He lives with a cat and they are very respectful of each other. Divo has never bitten another animal or human.
During loud noises such as thunderstorms, Divo is alert and trembles a little. His owner never took away his food bowl or bones so it’s unknown how he would react. When it’s a toy being taken away, Divo becomes playful. He isn’t bothered when pulled off of furniture, held from something he wants, or awoken from his slumber. He tolerates baths and nail clipping and loves being brushed. When someone unfamiliar approached Divo’s homes or humans, he is always friendly.
Divo’s owner describes him as friendly, affectionate, playful, and excitable. He has a medium to high activity level and when he’s ready to play enjoys fetch with balls and tugging on stuffed animals. His owner loves how much he plays balls and how he follows her around whenever she’s home. He’s been kept mostly indoors and sleeps on the sofa in the living room. He’s mostly house-trained and will rarely have accidents off his weewee pads. When he’s outside, he goes potty wherever he can pick up a scent. He is well-behaved when left home alone and has never been crate-trained. He knows come and go and loves his brisk walks on leash. He pulls lightly, but only when he sees something exciting. He eats Pedigree dry food, a small bowlful, three times a day. His favorite treats are Pedigree mini bones.
On intake, Divo allowed all handling.
Behavior
Divo hasn’t had a behavior evaluation yet but his medical behavior is blue (allows most handling)
