DVM Intake Exam: Estimated age: 10 Microchip noted on Intake? No Microchip Number (If Applicable): History : o surrender. Came in with 25212. Subjective: BAR-H. Long nails. Stilted hip gait. Observed Behavior – Easy to handle, nice. Objective: T = P = 100 R = eup BCS= 6/9 EENT: Eyes – nuclear sclerosis, PLR +, mild scleral injection, peri-ocular erythema, pedunculated eyelid mass bottom L eye. Ears clean. No nasal or ocular discharge noted Oral Exam: Mod tartar and tooth wear. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FS – spay scar seen MSI: Ambulatory x 4 but stilted pelvic gait and lean more to left/ grade 1/4 limpRRL. Ortho exam – does not allow extension of R hip but flexion okay, R stifle NSF,LRL – NSF, mild popping R tarsus. Skin free of parasites, no masses noted, healthy hair coat. Long nails – possibly impeding gait some. Corns and marked hyperkeratosis of back paws, no erythema seen. CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: Assessment: Geriatric. Long nails. R hip painful – r/o osteoarthritis. Eyelid mass OS – r/o adenoma Prognosis: Good esp with good response to arthritic tx. Plan: Rimadyl 75 mg 1 po sid x 5 days. SURGERY: Already spayed.
