Medical Summary DVM Intake Exam Estimated age: 5 yo intact male Microchip noted on Intake? negative History : o surrender but this person stated he got dogs from someone else a few days prior Subjective: QARH. Fleas. Very thin. Ate canned food well. Observed Behavior – Handleable and able to do full exam. Evidence of Cruelty seen – yes Evidence of Trauma seen – n Objective T = P = 100 R = eup BCS= 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild- mod gen tartar PLN: No enlargements noted H/L: NSR, NMA,MM pk CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male – 2 testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, Heavily matted cranial front legs and cadual back legs and tail, otherwise small generalized matts. Unkempt haircoat. Much flea dirt, some live fleas seen. Underweight – ribs and pelvic bones easily seen and palpated, waist very tucked. No fat noted. Minimal loss of muscle mass. BCS= 2/9. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: n Assessment: Moderately underweight. Matted. Fleas Plan: Routine intake w flea tx. Re-check weight in 5 days. Shave down tech check. Prognosis: Good SURGERY: Temporary waiver due to underweight. Recommend do after gaining 2-3 lbs.
