Medical Summary DVM Intake Exam (1406) Estimated age: 4 yo Microchip noted on Intake? N History : Stray, bit police officer but didn’t break skin. Requires sedation for any handling. Gave 0.6 ml telazol IM Subjective: BAR, overconditioned, superficial scars and wounds on hindfeet. Was seen to be bleeding from mouth on presentation but no wounds/lesions appreciated (likely from biting pole) Observed Behavior – barking and lunging in crate, will not allow handling Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 96 R = 20 BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: pink mm w/ CRT<2s, healthy adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male, both testicles palpate WNL MSI: Ambulatory x 4, skin free of parasites, no masses noted, multifocal abrasions esp around feet, otherwise healthy coat CNS: sedated exam, neuro not fully evaluated Assessment: apparently healthy Plan: Bite hold per DOH Prognosis: Good SURGERY: temporary waiver due to bite hold
