DVM Intake Exam. Estimated age: Approximately 8-10 Microchip noted on Intake? MC placed on intake. History : Owner surrender Subjective: BARH, panting, allows all handling. Objective: P = wnl. R = wnl. BCS = 7/9 EENT: Eyes clear, ears – AU: severe dark hyperpigmentation and lichenification of pinnae and canals, stenotic canals with yellow discharge, no nasal or ocular discharge noted. Oral Exam: heavy tartar at canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, no spay scar, swollen vulva MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt coat with mild matting CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: Externally normal Assessment: 1. Chronic, severe otitis AU 2. Dental disease. Prognosis: Fair to Good with treatment Plan: Clean ears, Claro AU. Recommend spay. Recommend follow up with veterinarian in future for dental cleaning and management of chronic otitis. SURGERY: Okay for surgery

Basic Information: Diamond is a 12 year old, small mixed breed female who was not spayed prior to coming into the shelter. She has no known health issues or injuries and has not seen a vet recently. She was surrendered due to lack of time to care for her.

For a New Family to Know : Diamond is described as being friendly, affectionate and relaxed. She likes to follow her owner around when she is home and has been an indoor dog. She eats dry food and sleeps anywhere in the home. She knows the command sit and walks on leash for exercise.
Behavior Notes: Diamond was relaxed and friendly during intake. She easily allowed for all handling.
