Medical Summary DVM Intake Exam Estimated age: 7 years History : owner surrender in the bronx-recently bit small child on the arm; transferred to MACC Subjective: BAR Observed Behavior – excited, overall friendly and good for restraint but muzzled for precaution Objective T =not taken P = WNL R = WNL BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: limited oral exam due to muzzle; mm appear pk/moist; mod staining on teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact; 2 normal descended testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted; overall coat looks healthy; small crusting callous on lateral aspect of L hock CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. Apparently healthy adult Plan no treatment need Prognosis: excellent SURGERY: Okay for surgery
