Medical Summary DVM Intake Exam Estimated age: 8 Microchip noted on Intake? no History : surrender Subjective: bar, hydrated Observed Behavior – friendly, timid, quiet, allows exam/treatment with minimal restraint.Objective P = 120 R = 20 BCS: 4/9 EENT: Eyes clear, hyperkeratotic pinna AU, no nasal discharge noted Oral Exam: grade 2 periodontal disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male castrated MSI: Ambulatory x 4, skin free of parasites, no masses noted, severe/chronic generalized alopecia and dermatitis. Skin is hyperkeratotic and lichenified in many areas. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: chronic, severe dermatitis as above: rule out infectious vs inflammatory vs autoimmune vs other. Skin scrape: negative for ectoparasites. Plan: treat with antibiotics/antifungals (malassezia?) recommend full derm evaluation including biopsies and allergan testing. Prognosis: good depending on response to medications. SURGERY: already altered.
