DVM Intake Exam; Estimated age: 9+yrs; Microchip noted on Intake? yes; History: Stray; Subjective: Dog is Q/BARH; Observed Behavior – shy/timid, allows all handling with minimal restraint; Objective ; T = NA; P = wnl; R = wnl; BCS 4-5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Grade 1/4 dental dz; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Female spayed, scar seen; MSI: Ambulatory x 4, skin free of parasites, ~1cm dermal mass left thorax, <5mm dermal mass right thorax, <1cm dermal mass left lateral stifle, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: not performed; Assessment: Senior healthy – dermal masses DDx: benign vs malignant neoplasia; Prognosis: Good; Plan: Possible owner reunification. Recommend FNA +/- removal of masses.; SURGERY: Already spayed
