Medical Summary DVM Intake Exam Estimated age: 9-10 Microchip noted on Intake? yes History : stray Subjective: quiet, alert Observed Behavior – friendly, allows handling and examination with little restraint Evidence of Cruelty seen – appears to have been abandoned/neglected Evidence of Trauma seen – no Objective P = 100 R = 20 BCS: 3/9 EENT: mild ocular discharge OU , ears clean, no nasal discharge noted Oral Exam: severe periodontal disease PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female, unsure if neutered MSI: Ambulatory x 4, severely matted and dirty haircoat, needs full grooming CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: geriatric with severe periodontal disease and matted coat.
