DVM Intake Exam: Estimated age: 10yrs History: O/S due to financial and time constraints Subjective: Dog is Q/BARH. Observed Behavior – immediately social – greets handler, vet and LVT; wagging tail; allows all handling with minimal restraint Objective: T = NA P = wnl R = wnl. BCS 6/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Grade 4/4 dental dz PLN: No enlargements noted H/L: NSR, Grade V/VI heart murmur, CRT < 2, Lungs clear, eupnic. ABD: Non painful, no masses palpated U/G: female intact, approx 4-5cm firm lobulated mammary mass right caudo-ventral abdomen; approx 1cm mammary mass left side as well MSI: Ambulatory x 4; grade 3/4 MPL LHL, grade 4/4 MPL RHL, skin free of parasites, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: not performed Assessment: Geriatric underlying – severe dental dz, Grade V/VI heart murmur, mammary masses, bilateral MPLs Prognosis: Fair-good long term depending on results of diagnostic tests and appropriate care Plan: Recommend rescue placement. Recommend full geriatric work-up on placement, including but not limited to: CBC/CS/T4 and UA, chest radiographs, and echocardiogram. If acceptable candidate for anesthesia based on BW and echo findings, should proceed with spay, removal of mammary masses for biopsy, and dental (will most likely need multiple extractions). SURGERY: Permanent waiver due to age and medical conditions
