**NEEDS PERINEAL HERNIA SURGERY ASAP**
5-6 Years Old, Small White and Tan Mix, Male, 10 lbs, Stray 10/14/17
Medical: 10/14/17: DVM Intake Exam; Estimated age: 5-6 years; Microchip noted on Intake? negative; History : stray ; Subjective: BARH; Observed Behavior – very sweet but extremely painful around perineal area; Objective P = wnl R = eupneic BCS 5/9 EENT: Eyes clear with tear staining ou, ears clean, no nasal discharge noted; Oral Exam: adult dentition with moderate dental tartar, no oral lesions noted; PLN: No enlargements noted; H/L: Sinus arrhythmia, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles; descended, severe left perineal hernia with fecal impaction, mild perineal hernia; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment:; Severe left perineal hernia with impacted feces; Mild right perineal hernia; Bleeding from rectum from fecal impaction; Plan: Continue to monitor while at BACC; Rec perineal hernia surgery; Start miralax 1/2 tsp PO BID x5d; Recheck daily to ensure defecating; Keep in medical; Manually removed impacted feces from rectum-bled during procedure; Rimadyl 4.4mg/kg PO SID x5d; Prognosis: Fair-needs surgical repair with 50% chance of failure; SURGERY: permanent waiver from ACC due to perineal hernia
Crouton has an extreme perineal hernia and needs surgery as soon as possible. He is unable to defecate and is extremely painful around that area.
