Medical Summary 06/03/17 17:30 DVM Intake Exam Estimated age: 5mos Microchip noted on Intake? History : stray found tied up in Crotona Park (per finder), admissions at BxACC reported that this dog vomited partialy digested food prior to transport Subjective: Observed Behavior – lethargic, wagging tail, ate small amount of cat food Objective T = 101.3 P = 160 R = 12 BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: adult dentition erupting, pink dry mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, perisstent skin turgor CNS: lethargic, no signs of neurologic abnormalities Rectal: formed light brown feces per rectum Assessment cardiovascular shock, severe dehydration primary gi (inx vs toxic vs FB obstruction vs other) vs secondary gi Plan parvo test neg IVF bolus 275ml, then maintenance at 40ml/hr CBC: Hct 66%, neutrophilia 27K with suspect bands (active inflammation), monocytosis Chem: Alb >6g/dl, TP 11g/dL, elevated Phos 10.3, elevated T Bili (8.1), elevated ALT (582) Rads: gas in cecum, feces in LI, loss of serosal detail overall, no obvious obstructive pattern 1.3ml Cerenia IV SID x3d 0.97 Ampicillin IV slow q6hr 1.2ml Baytril (100mg/ml) SQ q24hrs place with NH Prognosis: fair above BW and Rads consistent with severe dehydration (hi Hct and severe Albumin elevation), as well as infection (neutrophila and monocytosis with suspect bands neutrophilia), and possible sepsis (ie gi infection with bacterial translocation and septic shock). Severely elevated TBili and elevated ALT consistent with primar liver disease (ie congenital liver shunt) that could led to vomiting (due to toxin build up) and secondary dehydration. Alternatively, toxin ingestion can lead to chronic vomiting and dehydration, with possible elevations in liver values. SURGERY: Temporary waiver due to vomiting
