DVM Intake Exam; Estimated age: 10-12 years; Microchip noted on Intake? neg; History :stray; Subjective:QAR; Observed Behavior -docile, allows all handling; Evidence of Cruelty seen -overgrown nails with some embedding on RH; Objective ; BCS 4/9; EENT: OU-severe mucoid discharge, corneal edema and corneal pigmentation; corneas appear dry bilaterally; mucoid plug in corneal defect mid field OS with superficial neovascularization; Oral Exam:mm pk, tacky; CRT 2 sec; underbite; mild to moderate staining/tartar; PLN: No enlargements noted; H/L: eupnic, clear lungs; grade III-IV/VI focal L murmur; ABD: Non painful, no masses palpated; U/G: female intact; MSI: ambulatory x 4 with mild kyphosis of TL spine; diffusely thin hair coat with “rat tail”; nails all very overgrown with 3 on RH causing callous formation on digital pad; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal; Assessment; 1. KCS with corneal ulceration OU; 2. Geriatric; 3. Heart murmur; 4. Thin hair coat; 5. Overgrown nails with chronic irritation of pads; Prognosis:guarded to fair; Plan: -clipped nails -optimmune OU BID -ofloxacin eye drops OU TID-QID -CBC/chem -recommend cardiac work up including CXR, ECG, BP +/- echocardiogram after placement; SURGERY:permanent waiver due to geriatric

Basic Information: Cranberry is an older small mixed breed. She was brought in by the finder. There is not much behavior on her.
How is this dog around strangers? The finder was able to pick her up and hold her in their lap. She allowed to be pet all over her body by them.
Behavior Notes: upon intake, Cranberry was receptive to all handling. She allowed to be carried, pet, collared, and scanned. She had a calm body language.

Vet Notes: 11:02 AM: CBC/chemistry available for review; CBC-mild neutrophilia (13.67), moderate monocytosis (1.6), mild thrombocytosis; Chemistry-severe hyperglycemia (497), low creat (0.4), mildly elevated ALP (227), mildly elevated cholesterol (352), mild hypochloremia; A; Hyperglycemia-r/o diabetes mellitus vs DKA; Elevated ALP-r/o secondary pancreatitis vs other; P -place IVC, run normosol at 10 ml/hr -cerenia 0.4 ml IV SID x 3 days -NPH insulin 1 U SQ BID -urinalysis to r/o ketoacidosis
DVM notes 2/8: Diabetic with KCS and corneal ulcers; appeared lethargic and dehydrated on intake, started on IVF at 12 ml/hr overnight and humulin insulin: S/O -QAR/BAR, docile but much more energetic; begs for attention in cage! -good appetite with RC Glycoflex (diabetic diet) -mm pk, moist; CRT <2 sec -OU: severe mucoid discharge but cornea appear much brighter today -eupnic, clear lung sounds; focal grade III/VI L heart murmur -soft abdomen, mild borborygmus A 1. Diabetes mellitus 2. KCS, corneal ulceration P -continue IVF at 10 ml/hr through tomorrow then reassess rate -recommend BG curve in 10-14 days -continue meal feeding with glycoflex or other diabetic diet after placement

Look: 1. Dog holds gaze with soft eyes, soft body. Allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds.
Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Dog stands with tail low and wagging, and comes toward the Assessor in a friendly manner when the Assessor ceases moving.
Paw squeeze 1: 1. Dog does not respond at all for three seconds. Eyes are averted and ears are relaxed or back.
