DVM Intake Exam; Estimated age:8y; Microchip noted on Intake?n; History :stray; Subjective:appears blind. mydriatic eyes, no PLR, clumsy. became very upset at one point when he fell over during exam (disorientation i presume); Observed Behavior -seeks petting, wags tail; Objective ; T =; P =80; R =stertor; BCS 5/9; EENT: ou mydriasis, sclerosis, hyperkeratosis AU; Oral Exam:mod dental disease, unable to see back teeth; PLN: SMLN enlarged; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G:on testis felt; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: blind–appears very used to being blind. yeasty otitis. cryptorchid; Prognosis:good; Plan:mometamax au sid; SURGERY: Permanent waiver due to age
