02/26/16 11:59 BASIC INFORMATON Cookie is a 6 years female dog. Cookie was surrendered by her owner because the owner is blind and just can’t care for the dog. Cookie has never been to the vet and never has had any injuries. Cookie was given to the client as a gift. SOCILZATION Cookie is a very friendly dog. Cookie doesn’t have much outdoor experience so she does get shy just being outside. Cookie is very friendly to everyone around her even strangers. Cookie has never been around any animals. Cookie has been around a child around the age of 6. When cookie plays with the child she is very affectionate and protective. BEHAVIOR Cookie does have any behavior issues that would be issues inside of a home. Cookie goes to the bathroom on a wee pad and never has accidents anywhere else. Cookie isn’t bothered if her food bowl is touched or taken away. Cookie likes to get baths and enjoys being brushed. For a new family to know Cookie is a very sweet dog that loves affection. Cookie eats both wet and dry foods. Cookie does love water. She drinks about 4 bowls a day. Cookie loves toys and loves to sleep under the window. Upon intake Upon intake cookie was super sweet and friendly. Cookie allowed all handling.

Helper dog: A1065924 Cookie previously lived with 1 adult and has been around a child age of 6 years old. The owner stated when cookie plays with the child she is very affectionate and protective. The owner mentioned Cookie is house trained to use the wee wee pad in the house. It was reported by the owner cookie is very friendly towards strangers. At intake it was reported Cookie was very friendly and sweet, she allowed all handling. During SAFER Cookie came into the assessment room loose and wiggly, she was very friendly and affectionate towards the assessor. She allowed all handling items to be conducted without issue. Cookie was relaxed during the tag interaction, and during the resources portion of the assessment Cookie was easy to handle when engaged with the item. Cookie appears to be sociable, and did not display any behaviors concerns, so the department feels she can be suitable for an “Average” adopter. Look: 1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash, body soft. Squeeze 1 & 2: 1. Dog gently pulls back her paw. Toy: 1. Minimal interest in toy, dog smells, then turns away. Dog-dog: 2. Dog approaches helper dog with tail at spine level, body not stiff, ears relaxed, and lip neutral.

scan negative intact female weight: 51lbs slightly cloudy occular discharge clean ears dandruff to coat – missing hair around her tail and her hind legs (red, informed 1067) moderate staining to teeth along with gingivitis nails down to kwick lactating possibly in heat overall body condition is good very sweet girl – easy to handle BARH NOSF ADDENDUM: SMALL (APPROX. 1-2 CM) AREA OF ALOPECIA ON THE DORSAL PROXIMAL ASPECT OF THE TAIL. AREA IS ERYTHEMATOUS, APPEARS MILDLY THICKENED, HOWEVER NON-PAINFUL ALSO, CAUDAL HOCKS ARE ALOPECIC AND SEVERELY INFLAMED. NON-PAINFUL ON PALPATION AS WELL. THINNING HAIR ALONG LATERAL THORACIC AREA – POSS. SECONDARY TO RECENT GESTATION MODERATE FLAKING/DANDRUFF ON DORSUM. CLEANED WITH NOLVASAN, THEN APPLIED GENTASPRAY TO AFFECTED AREAS. WILL CONTINUE BID X 7 DAYS AND RECHECK INFLAMMATION SCHEDULED PATIENT FOR MEDICATED BATH WITH GROOMER IF TIME PERMITS
