**BLADDER STONES NEEDS FOLLOW UP VET CARE ASAP**

8 yr. spayed, stray, 9 pounds

Vet Notes:Hematuria and possible lameness noted on rounds board; History :surrender 10/5-hematuria noted by owner so started on baytril at BACC. 10/8 2 view AXR-one very large cystolith; CBC-wnl; Chem-mild elevation in BUN 30 (7-27); T4-wnl; Subjective: QAR. Hematuria noted on pee pad in cage. Normal bm. No c/s/v/d. Good appetite.Objective EENT: No nasal or ocular discharge noted.; H/L: No c/s. NMA. RR, Lungs C&E; ABD: tense, non painful, NMP; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat with matting between toes and overgrown nails. Haircoat shaved except between toes with mild pododermatitis. No pain elicited on ortho examCNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: ; Cystolith; Cataract OS; hematuria-rule out infectious vs inflammatory vs crystalluria vs other; Matted between toes with very mild pododermatitis; Overgrown nails; Plan: Continue baytril until 10/15; 2 view AXR-one very large cystolith, otherwise nsf; CBC-wnl; Chem-mild elevation in BUN 30 (7-27); T4-wnl; UA-unable to collect urine; Nail trim; Shave mats between toes; Telazol 0.15ml IM sedation for AXR; Recommend cystotomy to remove stone; Good prognosis

Basic Information: Cookie is a 6 year old small spayed female white dog who was surrendered by her owner due to personal problems. The owner stated that Cookie does have a cataract in her right eye.

How is this dog around children? She has spent time around a 5 year old boy and she was relaxed and affectionate. When playing with she is gentle.
How is this dog around other dogs? Cookie has not spent time around other dogs in the home so it is unknown how she will react.
How is this dog around cats? Cookie has also not spent time around cats so it is unknown how she will react.
Resource guarding: If someone was to touch her food treats or toys she is friendly.
Bite history: Cookie has never bitten a person or another animal.

Other Notes: Cookie is Wee-wee pad trained. Cookie’s owner stated that when being bathed she will snap and bite. staff bathed Cookie on her second day at the shelter and she was relaxed and did not show signs of aggression.

Medical Notes Cookie has a cataract in her right eye.
For a New Family to Know Owner described Cookie as friendly affectionate and playful. When at home she will follow her owner around. She likes to play with balls and stuffed toys. Cookie has been kept mostly indoors and eats dry food. When left alone in the house she is well behaved. She has been crated and does well for a few hours at a time. Cookie knows how to come when called and to get down. She is used to brisk walks on the leash. When off the leash she might run away.
