DVM Intake Exam; Estimated age: 7-9yrs based on PE and dentition ; Microchip noted on Intake? scanned negative by LVT on intake. MC placed on intake. History : stray brought in. Subjective / Observed Behavior – QAR, allows all handling. tense with tail tucked during exam. Objective ; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: all mandibular incisors and #104 worn down; dc 2/5; pd 2/5 ; PLN: No enlargements noted; H/L: No murmur ausculted; CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: spayed female. spay scar seen. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: grossly normal. Assessment; geriatric ; dental disease ; Prognosis: excellent ; Plan: adoption! SURGERY:already spayed.
