07/23/16 09:48 Basic Information Cody is a 10 year old male Maltese who is being surrendered due to conflicts between pets in the household. Owner has had Cody for five years. Cody was seen by a vet back in April. Cody had ear mites that were treated but came back. Socialization Cody is friendly around strangers and has lived with kids ages 7 through 10. Around the kids Cody was relaxed, playful, respectful, and tolerant of their handling. He plays in a gentle manner with adults as well as kids. Cody lived with two Shih Tzu’s ages 2 and three. Cody was tolerant of the two other pets in the household. Owner stated the Shih Tzu’s were bullying Cody. Cody has never been around cats. He has never bitten a person or animal. Behavior Owner stated the only behavior issue she could think of is that he howls at night. Cody is not bothered by sudden or loud noises. Owner states if you try to touch Cody’s bowl while he is eating he will try to eat faster. Owner stated she does not give Cody rawhide or treats. Cody is not bothered if you take his toy away, is pushed off the furniture, being picked up, disturbed while sleeping, brushed, having his nails trimmed, or if someone unfamiliar comes to the door. Owner stated Cody likes to take baths. For a New Family to Know Cody is described as friendly and independent with a medium activity level. Owner favorite thing about Cody is that he likes to jump on his hinds legs. At home he will follow you around and likes to play with stuffed toys. Cody is kept indoors only and sleeps in a crate. He eats wet food only. Owner stated he does not like beef it has to be chicken. He eats pedigree and beneful. Client states Cody will potty outside on cement. Owner stated not to take him on the grass because his skin is sensitive and will cause a rash. When left t home Cody is well behave, Cody has never been left in a yard. He is crate trained but after 8 hours or o he will want o come out. Cody know commands like sit and stay. He likes slow walks on the leash and does not pull. Owner stated she does not walk him off the leash. Cody likes water a lot. Behavior during Intake Cody tolerated all handling during intake. No signs of aggression was displayed.

Cody walks calmly on leash. He was sociable toward the handler during the assessment – soft body. He was calm and relaxed during handling. Cody had a low body position during the tag test, but followed the assessor at the end of the leash. He was not interested in toys. Cody did not show any concern during the behavior assessment. The behavior department feels that he can go to an Average home. Look: 2. Dog pulls out of Assessor’s hands each time without settling during three repetitions. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash, body low and a bit fearful. Squeeze 1: 1. Dog gently pulls back his paw. Squeeze 2: 1. Dog gently pulls back his paw. Toy: 1. No interest.
