Medical Summary DVM Intake Exam Estimated age: 16 years History : stray Subjective: Observed Behavior – tense but tolerated exam Evidence of Cruelty seen – matted, dirty coat Evidence of Trauma seen – no Objective BCS 4/9 EENT: OU- corneal opacity, negative menace response; OS- buphthalmic , AU- otic hair, no nasal discharge noted Oral Exam: severe dental calc, missing teeth, tongue protruding to side PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Tense, no masses palpated U/G: MI, 2 descended testicles MSI: Ambulatory x 4, skin free of parasites, approx 2 inch firm cutnaneous mass at right forelimb; approx 2 cm erythematous cutaneous mass at righ tlateral thigh; approx 1.5 cm soft pedunculated mass at ventral perianal area; long nails; matted and dirty coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe dental disease blind, corneal opacity and buphthalmia- R/O glaucoma vs KCS vs other matted cutaneous masses- R/O benign vs malignant Plan signed up for bloodwork 7/12/17 recommend STT, tonometry, and fluoroscein stain OU Prognosis: guarded SURGERY: Permanent waiver due to age and possible underlying conditions
